Registration Information
Welcome and Thank-You for choosing Woodbridge Play-Care. We hope that the information provided below will assist you in helping your child make a smooth transisition into our center. Prior to First Day of Enrollment:
- Completed Enrollment Form(Front& Back)
- State Physical Form
- Proof of Identity(i.e. Birth Certificate)
- Signed Behavior Contract
- Emergency Hospital Form
- Payment
What toBring on the First Day of Enrollment:
New Pre-Schoolers
-A clean Sheet & Blanket
-2 sets of extra clothes (LABELED)
-Diapers and wipes if necessary
-Breakfast for children arriving before 8:15 AM
New School-Agers
-Notice of Bus Pick-up or Bus Departure and where to meet the van if transportation is to be provided by the center
-Inform the staff of the teacher's name and grade your child attends
-A book bag or backpack to carry their belongings
-Breakfast for children arriving before 7:45 AM
